Summary of Duties

- Review daily all Tory Trauma Program trauma and neurosurgery patients in the ED and off-service beds.
- Coordinate daily care planning and communication with MRP services, consulting teams, patients, and families to support timely discharge and follow-up.
- Facilitate repatriation within the Tory Trauma Program through identification, communication, coordination, and follow-up.
- Complete and coordinate applications to support discharge and seamless transitions to the appropriate care setting.
- Proactively identify and address barriers to care, coordinating services, imaging, and resources to optimize recovery and reduce delays.
- Collaborate with Patient Flow, OR, ICU, Acute Care, and other hospital departments to identify discharge priorities, placement needs, and opportunities to improve patient flow.
- Maintain regular communication with patients and families, providing updates and supporting them throughout their care journey.
- Engage in trauma and neurosurgery quality improvement initiatives and support ongoing practice improvement.

Qualifications/Skills

- RN with 3+ years of frontline trauma and neurosurgery experience




- Demonstrated leadership experience; TL/PCC experience or DNLP preferred.
- Strong knowledge of complex discharge planning, repatriation, rehabilitation applications, OH@Home,SB2H, discharge summaries, and care coordination required
- Able to work effectively across all Tory Trauma Program care areas, including ICU.
- Demonstrated patient and family advocacy and commitment to patient-centred care.
- Strong understanding of operational priorities, patient flow, resource utilization, and competing demands.
- Demonstrated conflict resolution and relationship management skills, with the ability to navigate challenging situations respectfully and effectively.
- Demonstrated commitment to continuing education, career development, and ongoing learning.
- Demonstrated ability to adapt to changing priorities and manage multiple demands.
- Able to work independently and collaboratively in a fast-paced, complex environment.
- Excellent interpersonal, communication, critical thinking, and problem-solving skills.
- Excellent attendance and reliability.
