Shape the future of palliative care as the Clinical Coordinator at Kenora Chiefs Advisory. This full time position is dedicated to managing care for patients with chronic diseases and end-of-life situations in Ontario. As part of the Primary Care Team, you will lead the early identification and comprehensive care planning process.

By collaborating closely with elders and health professionals, you will ensure that care is both culturally safe and responsive. Your contributions will strengthen community ties and facilitate smooth transitions between different care settings. Key Responsibilities:

- Identify community individuals who need palliative approaches
- Coordinate care and case management with allied Health Professionals
- Provide navigation support for patients and caregivers




- Deliver education on available resources and care practices
- Prepare detailed evaluations and reports per leadership direction Requirements:
- University degree or college diploma as a Registered Nurse
- Strong time management and organization skills
- Familiarity with Anishinaabe culture and Ojibway language is beneficial
- Must possess a valid driver’s license and be open to travel
