The John Noble Home is looking for Temporary Full-Time Resident Relations Coordinator
POSITION SUMMARY:
The Resident Relations Coordinator plays an significant role in supporting residents and families throughout the admissions process and during their transition and residency at John Noble Home. The position is responsible for coordinating admissions, supporting resident and family relations, providing psychosocial support and advocacy, and connecting residents and families with appropriate internal, community, and external resources.
Working collaboratively with residents, families, the interdisciplinary team, placement partners, and community service providers, the Resident Relations Coordinator promotes a resident-centred approach and supports a positive transition and experience for residents and their families.
DUTIES AND RESPONSIBILITIES:
- Coordinate and support the admissions process, including communication with prospective residents, families, placement partners, and internal departments.
- Support residents and families throughout the transition into long-term care by providing information, resources, guidance, and follow-up.
- Develop and maintain positive relationships with residents and families and respond to questions, concerns, and requests in a timely and compassionate manner.
- Provide psychosocial support, advocacy, and assistance related to adjustment, transitions, loss, family concerns, and other resident needs.
- Facilitate communication between residents, families, staff, healthcare professionals, and external service providers.
- Assist with addressing and resolving resident and family concerns and facilitate appropriate follow-up.
- Participate as a member of the interdisciplinary team and contribute to resident-centred care planning and service delivery.
- Connect residents and families with appropriate internal, community, and external resources.
- Maintain accurate, timely, and confidential documentation and records.
- Support resident and family communication throughout the admission and transition process.
- Participate in committees, quality improvement initiatives, and other organizational activities as required.
- Maintain confidentiality and comply with applicable legislation, policies, and procedures, including privacy requirements.
- Perform other related duties as assigned.
QUALIFICATIONS:
- Bachelor’s degree in Social Work, Social Sciences, Human Services, Psychology, Gerontology, Health Administration, or a related field required.
- Master’s degree in Social Work or a related discipline considered an asset.
- Registered Social Worker (RSW) designation considered an asset.
- Minimum two (2) years of related experience in social services, healthcare, long-term care, community services, admissions, or a related environment.
- Experience working with older adults and families considered an asset.
- Knowledge of long-term care and community resources considered an asset.
- Strong communication, interpersonal, advocacy, and conflict-resolution skills.
- Demonstrated ability to work collaboratively with residents, families, healthcare professionals, and community partners.
- Strong organizational and time-management skills, with the ability to manage multiple priorities and sensitive matters.
- Demonstrated ability to exercise sound judgment, discretion, and professionalism when dealing with confidential and sensitive information.
- Proficiency with Microsoft Office and electronic documentation systems.
