The APP collaborates with hepatology, transplant surgery, pharmacy, nutrition, and care teams to manage patients across the transplant continuum.
The role includes pre-transplant evaluation, peri-transplant management, and long-term post-transplant follow-up, delivering secure, evidence-based care within the APP scope
