ByWard Family Health Team is seeking an experienced Nurse Practitioner to join our interdisciplinary primary care team in a permanent, full time role. You will take ownership of an established patient panel of approximately 1,700 patients with access to a broad interdisciplinary team for support. You will independently assess, diagnose, treat and manage a wide range of health conditions, order investigations, prescribe medications, provide preventive care, and coordinate referrals within a fully #J-18808-Ljbffr
