As part of Ontario Shores, you will provide comprehensive assessments and patient-centered treatment within the Geriatric Transitional Outreach Clinic.

Your work will help address complex mental health issues and improve care for patients aged 65 and older. Collaborating with a dedicated team, your expertise will guide treatment plans and foster effective family communication. Key Responsibilities:
Lead psychiatric, medical, and ongoing cognitive assessments
Safely prescribe and monitor therapeutic medications
Develop and implement treatment and rehabilitation plans




Educate families on diagnosis and care options
Engage with community partners for integrated patient support Requirements:
Current registration as RNEC with no practice restrictions
At least 2 years of Geriatric/Mental Health experience
Graduate degree preferred
Knowledgeable in mental health and geriatric nursing
