Job Overview

We are seeking a compassionate and dedicated Dementia and Mental Health Specialist to join our dynamic team. In this vital role, you will provide expert support and personalized care to individuals experiencing dementia, mental health challenges, and related cognitive conditions. Responsibilities

- Develop and implement individualized care plans tailored to each client’s unique conditions, preferences, and goals.
- Collaborate with multidisciplinary teams including nurses, physicians, social workers, and family members under guidance of lead specialist
- Provide direct therapeutic interventions, including counseling, behavioral support, and education on managing dementia and mental health symptoms
- Educate families and caregivers on effective communication techniques, safety measures, and daily routines to support ongoing well-being.
- Maintain detailed documentation of progress notes
- Provide transportation, communication assistance, reporting and follow through of appointments
- Provide activity and engagement suitable to client needs and preferences




- Work from a variety of settings including but not limited to private home, retirement setting or long term care
- Must have own car and comfort transporting clients, and any equipment they may require ex walker, wheelchair etc

Skills

- Proven experience working within a variety of settings with a focus on mental health or neurological conditions.
- Solid background in healthcare fields with knowledge of medical terminology relevant to mental health and dementia care.
- Expertise in case management practices to coordinate services effectively
- Excellent communication skills for engaging with clients, families, and interdisciplinary teams in a clear and empathetic manner.
- Ability to handle complex situations with professionalism, patience, and active problem-solving skills.
- Knowledge of best practices in dementia care, mental health treatment protocols, and safety procedures.
