Job Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. We are a personal care home that offers care to patients with dementia. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments.

This role requires a strong foundation in nursing practices, excellent communication skills, and the ability to work effectively within multidisciplinary teams. The LPN will be responsible for administering medications, monitoring patient health, and supporting overall patient well-being in compliance with healthcare regulations and standards.

Responsibilities

- Deliver comprehensive patient care tailored to individual needs, including medication administration and vital sign monitoring
- Support patients with activities of daily living, including mobility assistance and hygiene care
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement care plans
- Manage patient documentation related to medical history, treatment plans, and progress notes




- Educate patients and their families on health management strategies, including dementia care, pain management, and hospice care
- Ensure compliance with safety protocols and infection control standards in all patient interactions

Experience

- Prior experience in nursing home settings, hospitals, or assisted living facilities is highly preferred
- Experience with acute care, dementia care, pediatrics, or senior care is advantageous
- Familiarity with medical terminology and medication administration protocols is essential
- Background in home care or hospice environments adds value to the role
- Laboratory experience or skills in phlebotomy are beneficial
- Knowledge of case management practices and pain management techniques enhances patient support capabilities
- Previous experience working with diverse patient populations including children and elderly individuals is a plus

Job Types: Part-time, Fixed term contract, Seasonal Pay: $20.00-$50.00 per year
