The Espanola & Area Family Health Team is currently recruiting Permanent Full time

Registered Dietitian.

The successful candidate will have familiarity with the health issues of the community and will work collaboratively with other primary care providers to improve the health of the patients accessing services through the Family Health Team.

Responsibilities:

o Assess clients’ nutritional status, dietary habits, health needs, and nutrition-related risk factors.

o Develop individualized nutrition plans and provide practical guidance on food choices, portions, and meal planning.

o Support the prevention and management of chronic conditions through evidence-based nutrition interventions and client education.

o Support clients in setting goals, overcoming barriers, and maintaining healthy eating behaviours using effective behaviour-change strategies.

o Collaborate with the interdisciplinary healthcare team to integrate nutrition services into coordinated, patient-centred care.

o Provide specialized nutrition counseling for individuals or groups with unique needs, specific medical conditions or dietary restrictions.

o Assess and monitor individuals' progress in meeting their nutritional goals.

o Adjust nutrition plans as needed and evaluate the effectiveness of dietary interventions.

o Engage in community outreach activities to promote nutrition education and preventive healthcare.

o Participate in health promotion programs to address broader public health issues related to nutrition.

o Maintain accurate and up-to-date records and statistical data related to nutritional assessments, interventions, individual progress, program performance, and quality improvement.





o Ensure compliance with healthcare regulations and standards in documenting nutrition care.

Qualifications:

- Current registration in good standing with the College of Dietitians of Ontario
- Demonstrated experience working in community setting or primary care setting
- Ability to problem solve effectively and independently
- Demonstrated knowledge of the role of interdisciplinary health professionals and

the ability to work effectively within a collaborative team
- Strong interpersonal and communication skills
- Experience working in an electronic medical record system is an asset
- Fluency in English and French is an asset
- Good organizational skills and time management skills. Demonstrated ability to

manage multiple tasks, prioritize workload.
- Knowledge of Consent and Release of Information legislation
- Knowledge and experience with nutritional needs for diabetic clients (chronic

disease management)
- Bilingualism (English and French) is preferred

The wage range for this role reflects a 5-step grid from $36.359/hour to $41.069/hour. Starting wage range will be determined based on the successful candidate’s qualifications and relevant experience.
