We’re Hiring: Social Worker — 0.8 FTE

$74,000–$77,673 annually, commensurate with experience

WMCFHT is looking for an experienced Social Worker to join our interdisciplinary primary care team.

This is a 0.8 FTE position designed with flexibility in mind. We’re looking for someone who enjoys working independently, values collaboration, and wants the opportunity to use their clinical expertise in a role that can evolve with their strengths, interests, and the needs of our patients.

You’ll work alongside physicians and other health care professionals to provide counselling, psychosocial assessment, care coordination, advocacy, and connection to community resources. You’ll also have opportunities to contribute to programs, group services, quality improvement initiatives, and new ideas that enhance patient care.

What You’ll Be Doing In this role, you will:

- Provide individual, couples, family, child, and youth counselling based on patient needs.
- Provide assessment and therapeutic support related to mental health, family concerns, relationship challenges, work-related stress and conflict, eating disorders, substance use, and other psychosocial concerns.
- Work collaboratively with physicians and allied health professionals to develop and support patient-centred care plans.
- Provide evidence-informed interventions, including approaches such as Cognitive Behavioural Therapy (CBT).
- Support patients and families in navigating health, social,



and community services.
- Connect patients with appropriate community and specialized resources, including mental health, crisis, and long-term care services.
- Support transitions in care and discharge planning where appropriate.
- Advocate for patients and families and help remove barriers to accessing care.
- Participate in interdisciplinary case discussions and collaborative care planning.
- Facilitate groups, workshops, and educational initiatives as appropriate.
- Contribute to the development, implementation, and evaluation of programs that respond to patient and community needs.
- Participate in quality improvement initiatives and help measure and communicate patient outcomes.
- Maintain accurate and timely documentation in the EMR.
- Maintain professional knowledge and skills through ongoing education and development.
- Contribute to a positive, respectful, and collaborative team environment.

Required

- Master’s degree in social work (MSW).
- 3–5 years of clinical social work experience.
- Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Clinical experience and demonstrated skills in:
- Family counselling
- Couples counselling
- Child and youth counselling
- Work-related stress
