Staff Development and Resident Care Coordinator
St. Clair O’Connor Community Inc. is a non-profit partnership between the broader community and Danforth Mennonite and Toronto United Mennonite Churches providing a range of accommodations and a continuum of care, a variety of life-affirming activities, and programs for seniors in the surrounding neighborhoods. SCOC seeks to be a caring, intergenerational community, which nurtures the health and well-being of its residents.
SCOC is presently accepting applications for a permanent full time Staff Development and Resident Care Coordinator.
Hours: 9:00am to 5:00 pm, Mondays to Fridays, and may include weekends and holidays as needed.
Salary: 45,000 – 55,000 annually
Position Summary
Reporting to Director of Health Services, the Staff Development and Resident Care Coordinator is responsible for high-quality resident care while contributing to staff education, development, and ongoing competency. This role oversees coordination of resident care needs, staff training, communication, and quality improvement initiatives. The successful candidate will be a positive role model and trusted resource for frontline staff, helping promote consistent care practices, professional growth, and a culture where residents are supported to live with dignity, choice, independence, and purpose.
Key Responsibilities
· Support and coordinate the delivery of high-quality, person-directed care that achieves meaningful outcomes for residents
· Promote continuity of care through effective communication among PSWs, nurses, families, and other care providers
· Identify care gaps, follow-up needs, and opportunities to improve resident outcomes
· Participate in care conferences, team meetings, and resident-related discussions as required
· Ensure resident dignity, privacy, safety, choice, and independence are always maintained and aligned with person-directed living
· Support and ensure consistent care practices across shifts and departments
· Promote best practices across key areas of resident care, including falls and restorative care, infection prevention and control, skin and wound management, incontinence care, responsive behaviours, medication management, and pain and palliative care
· Assist with the selection, hiring, orientation,
and onboarding of new PSWs and care staff
· Provide ongoing mentorship, coaching, and support to PSWs and other frontline care staff
· Support competency assessments and identify individual and team learning and development needs
· Facilitate and assist with in-service education, skills reviews, refresher training, and other learning opportunities
· Support staff in maintaining compliance with organizational policies, procedures, and applicable standards
· Encourage a positive learning environment and support continuous professional growth and development
· Act as a positive role model and resource for PSWs and other frontline care staff
· Support the implementation of the organization’s progressive discipline process and participate in performance management, performance reviews, and staff termination process, as required
· Identify trends, concerns, and opportunities for improvement related to resident care and staff practices
· Collaborate with nursing and leadership to implement quality improvement initiatives
· Assist with audits, reviews, incident follow-up and other quality initiatives as assigned
· Develop, coordinate, and participate in Core Programs, Continuous Quality Improvement, Joint Occupational Health and Safety, and other committee meetings as assigned
· Support initiatives that promote resident safety, quality of care, and compliance with organizational and legislative requirements
· Provide payroll and scheduling support as needed
· Promote teamwork, respectful communication, and a culture of person-directed care
· Practice and promote The Eden Alternative person-directed philosophy of St. Clair O’Connor Community
· Communicate courteously and professionally with residents, families, staff, visitors, and external care providers
· Maintain confidentiality and appropriate professional boundaries at all times
· Actively involved in own professional growth and development
· Perform other duties as assigned
Qualifications
· Post-secondary certificate, diploma, or degree from a healthcare-related program; Personal Support Worker (PSW) certificate preferred
· Current First Aid and Basic Cardiac Life Support (BCLS) certification
· Previous experience working in a long-term care, retirement, community, or other resident-care environment
· Strong knowledge and understanding of person-directed care principles and resident safety
· Training and experience in The Eden Alternative philosophy of care, or willingness to complete required training within 6 months of hire
· Experience mentoring, coaching, training, or supporting other staff is an asset
· Excellent communication, organization, interpersonal, and problem-solving skills
· Strong leadership and relationship-building abilities
· Demonstrated ability to work collaboratively with residents, families, nurses, PSWs, healthcare professionals, and other team members
· Demonstrated ability to maintain confidentiality and professional boundaries
· Ability to manage competing priorities, strict timeframes, and deadlines effectively
· Strong computer skills and proficiency with relevant electronic systems
As a condition of employment, selected candidates will be required to provide a satisfactory Police Reference Check (PRC) with vulnerable sector screening, tuberculosis screening, and proof of immunization in accordance with the Fixing Long-Term Care Act, 2021 (O. Reg 246/22).
