We are seeking experienced and compassionate Personal Support Workers (PSWs) who are committed to providing exceptional care, including support for clients receiving palliative and end-of-life care.

The Role

As a Personal Support Worker with Modern Angels Healthcare, you will provide individualized support based on each client's care plan, preferences, abilities, and changing healthcare needs.

You may support clients who are aging in place, recovering from illness or surgery, living with dementia or chronic conditions, or receiving palliative and end-of-life care.

Your role is not simply to complete tasks. We expect our caregivers to provide presence, comfort, dignity, reassurance, and compassionate companionship to both clients and their families.

Key Responsibilities

Responsibilities may include

- Assisting with bathing, grooming, dressing, toileting, continence care, and other activities of daily living
- Supporting safe mobility, transfers, repositioning, and ambulation
- Providing meal preparation, feeding assistance, and hydration support
- Providing medication reminders within the PSW scope of practice and established care plan
- Providing companionship, emotional support, and meaningful engagement
- Supporting clients living with dementia, Alzheimer's disease, Parkinson's disease, and other chronic conditions
- Supporting clients following hospitalization, surgery, illness, or rehabilitation
- Monitoring and reporting changes in a client's physical, cognitive, emotional, or behavioural condition
- Following individualized care plans and communicating concerns promptly to the Modern Angels care team
- Completing accurate shift documentation and providing clear end-of-shift updates
- Maintaining client privacy, confidentiality, dignity, and independence




- Communicating professionally and compassionately with clients, families, nurses, care managers, and other members of the healthcare team

Palliative & End-of-Life Care PSWs assigned to palliative clients may be responsible for:
- Providing compassionate personal and comfort care during advanced illness
- Supporting clients who wish to remain safely and comfortably at home
- Assisting with repositioning, skin care, mouth care, hygiene, and other comfort measures
- Providing toileting and continence care with dignity and sensitivity
- Assisting with nutrition and hydration as directed by the client's plan of care
- Recognizing and promptly reporting changes in pain, breathing, alertness, mobility, intake, output, or overall condition
- Supporting a peaceful, calm, and respectful workplace
- Providing reassurance and emotional support to clients and family members
- Working collaboratively with nurses, physicians, palliative care teams, hospices, and family caregivers
- Respecting the client's wishes, cultural practices, spiritual preferences, and goals of care
- Providing compassionate presence during the final stages of life

Experience supporting individuals receiving palliative, hospice, or end-of-life care is strongly preferred. Qualifications The successful candidate must have:
- Personal Support Worker Certificate from a recognized Ontario program
- Previous PSW experience in home care, retirement living, long-term care, hospital care, or community healthcare
- Experience providing palliative or end-of-life care is an asset
- Strong understanding of person-centred and family-centred care
- Excellent communication and interpersonal skills
- Compassionate, calm, respectful, and professional approach
- Strong observational skills and ability to recognize and report changes in a client's condition
- Ability to work independently while remaining accountable to the care team




- Excellent judgment and reliability
- Ability to follow care plans, policies, infection prevention practices, and safety procedures
- Current CPR/First Aid certification
- Current Vulnerable Sector Check
- Required health and immunization documentation in accordance with organizational and client-site requirements

Experience with dementia care, complex care, mobility assistance, post-operative care, and palliative care is considered an asset.
