The person will provide strategic vision and management for the comprehensive requirements that will require internal and external collaboration to ensure that Minnesotan's maintain their continuity of care. This includes people who need new services or care when impacted by a provider, agency, or similar transition, service disruption or termination, as well as individuals experiencing (or at risk of) institutionalization.The position is embedded within the Transitions, Tribal & Transformations Division of the Aging and Disability Services Administration. While this position is state‑funded, the division is primarily funded by the federal demonstration grant Money Follows the Person. This position will support both the demonstration and permanent functions and projects of the agency, administration, division, and demonstration grant.All DHS Leaders are expected to demonstrate and develop the six (6) competency themes from the Enterprise‑wide Leader Competency Model:Leading SelfLeading OthersLeading the OrganizationLeading Outside the OrganizationGetting Work DoneMinimum Qualifications
Demonstrated leadership experience*Leadership experience can include:Experience as a supervisor, leadworker, team lead, project lead;
