We are looking for people who, like us, believe the work we do has a positive impact on their community and that together we can continue supporting our Residents and their families.As a Registered Practical Nurse (RPN) leader of our frontline care team, the Resident Care Coordinator - Rehab Lead plays a vital role in the Home. Your voice, ideas, and leadership help shape daily care practices, while your presence and energy contribute directly to creating meaningful moments and smiles for our Residents who live here.If you were working with us as a Resident Care Coordinator - Rehab Lead, here are some of the things you would have done last week:Led the Rehabilitation program using your knowledge and understanding of the principles of rehab programs – e.G. ambulation, mobility, communication, fall prevention, dining, ADLs, Active ROMProvided clinical leadership in your assigned resident home area as a Registered Practical Nurse in good standing with the College of Nurses of OntarioLed and supported the front-line PSW team by clearly communicating organizational goals, priorities, vision, and policies to achieve the best possible resident outcomesEnsured Resident-Directed Care principles were consistently applied in daily care practicesOversaw clinical programs and ensured compliance with Long-Term Care legislation, Ministry of Health requirements, Health & Safety standards, and nursing standards of practiceParticipated in Home-wide quality improvement activities, including data collection, analysis, reporting,



and contributing to Quality Improvement Plans (QIPs) in alignment with HQO principlesCollaborated with the Leadership Team on educational needs within the Home and your assigned resident areaSupported, oriented, and onboarded newly hired PSW Team Members, including training on PointClickCare and Point of Care documentation practicesCoordinated and led PSW team meetings and daily home-area huddles to promote communication, accountability, and teamworkManaged PSW performance-related matters, including coaching, performance appraisals, discipline, and participation in hiring or terminations as requiredActed as a point of contact for resident and family concerns, addressing issues proactively to prevent escalationAdvocated for Resident Rights and supported the development and ongoing review of individualized Resident Care PlansMaintained accurate and up-to-date documentation related to residents’ conditions, progress, care instructions, and evaluationsParticipated in Care Conferences and worked closely with a multidisciplinary team to support resident care planningKept current with best practices, emerging trends in gerontology, and nursing practice to continuously improve care deliveryDemonstrated positive, hands‑on leadership by example while working collaboratively with the broader leadership teamAdjusted work hours as needed to support the operational needs of the Home,

While we don't take ourselves seriously, we take our jobs seriously and love to have fun with each other and the Residents!To be hired into this role, you must have:Registration with the College of Nurses of Ontario in good standingVulnerable Sector Screening (VSS) check completed within the past 6 monthsTB 2 Step updated within the last yearExperience in geriatric nursing with a genuine interest in all aspects of Long Term CareKnowledge of LTC legislation, MOH compliance and Health and SafetyExcellent interpersonal, communication, organizational and team skillsDemonstrated experience and passion for positive leadershipComputer skills (knowledge of Point Click Care, Microsoft Office)Proficient in HQO principles and activities related to quality improvement activities including data collection, analysis, reporting, building QIPS.#J-18808-Ljbffr
