Rideau Community Health Services in Brockville seeks a Registered Nurse for a full time role spanning primary care clinics and diabetes education. The RN provides direct patient care, health education, and care coordination to individuals with complex needs in a multidisciplinary setting.

You will lead specialty clinics, support the primary care team, and collaborate with dietitians to deliver community-based diabetes education and management programs across Lanark, Leeds and Grenville.
