As a full time Nurse Practitioner, you will report to the Senior Manager of Child, Youth and Family Services, contributing to a racially informed care model. Engage in interprofessional collaboration and facilitate group sessions, while ensuring continuity of care through comprehensive assessments.Key Responsibilities:
- Evaluate and monitor patient health using best practices
- Support interprofessional team consultations and resource management




- Lead and coordinate educational group sessions
- Execute crisis management and risk assessments
- Document patient progress and treatment plansRequirements:
- Registered Nurse in Extended Class (RN(EC)) with Ontario
- Proficiency in mental health care practices
- Exceptional communication and leadership skills
- Familiarity with the Mental Health Act
