The Community Care

Nurse, reporting to the Health Manager, plays a crucial role in various health programs. Core responsibilities include providing nursing assessments, wound care, and ensuring culturally appropriate programs for Elders and those with chronic illnesses. This role requires a commitment to maintaining client confidentiality and effective documentation. Key Responsibilities:
Assist with home care for clients with health conditions
Manage a comprehensive inventory of medical supplies
Ensure culturally respectful approaches are applied
Conduct evaluations and maintain teaching documentation
Collaborate with community services and health specialists Requirements:
LPN certification required; RN preferred
Current registration with BCCNM required
Valid Class 5 Driver’s License essential
Experience in wound care and chronic disease management
