We are seeking a motivated, compassionate, and highly organized Registered Nurse (RN) to join our team in a unique hybrid role combining Care Management, Special Projects, and Marketing Support.

This position is ideal for an RN who enjoys direct client and family interaction, care coordination, teaching, community engagement, presentations, relationship building, and contributing to the continued growth of a healthcare organization.

Position Overview The successful candidate will assist with the assessment, coordination, and ongoing management of client care while also supporting special projects, community initiatives, marketing activities, presentations, and organizational development.

This individual must be clinically knowledgeable, professional, self-directed, creative, organized, and comfortable interacting with clients, families, healthcare professionals, community organizations, and referral partners.

Care Management Responsibilities

- Complete initial and ongoing nursing assessments.
- Develop and update individualized, person- and family-centred care plans.
- Monitor clients for changes in condition, safety concerns, or changing care needs.
- Use sound nursing judgement to identify concerns and determine when escalation or additional assessment is required.
- Communicate regularly with clients, families, caregivers, and healthcare professionals.
- Provide guidance and support to PSWs, nurses, companions, and other members of the care team.
- Participate in client and family meetings.
- Follow up with families regarding quality of care and changing client needs.
- Support clients transitioning home following hospitalization, surgery, illness, or rehabilitation.
- Support clients aging in place and those receiving additional care in retirement homes, long-term care, and hospital settings.
- Review clinical documentation and assist in maintaining accurate and timely client records.
- Assist with follow-up related to incidents, falls, medication concerns, changes in condition, or hospitalizations.
- Work collaboratively with Care Coordinators and other members of the Modern Angels Healthcare team.
- Help ensure client care plans are being followed appropriately.

Education & Clinical Support
- Provide education, coaching, and clinical guidance to healthcare staff.
- Assist with orientation and training of PSWs, nurses, and other caregivers.
- Teach clients and families regarding care needs, safety, disease management, medication considerations, mobility, skin integrity, dementia care, and other relevant health concerns.




- Assist with staff education sessions and clinical presentations.
- Support ongoing staff competency and professional development initiatives.
- Help develop educational resources, care guides, and clinical materials.

Special Projects The successful candidate will also assist with special projects that support the continued development of Modern Angels Healthcare.

Responsibilities may include

- Coordinate and support assigned healthcare and organizational projects.
- Assist with the development and implementation of new programs and services.
- Research healthcare trends, community needs, and potential service opportunities.
- Assist with quality-improvement initiatives.
- Support the development of policies, procedures, educational materials, and operational resources.
- Participate in community health initiatives and organizational events.
- Help coordinate projects involving hospitals, retirement residences, long-term care facilities, pharmacies, clinics, and community organizations.
- Track project activities, timelines, deliverables, and follow-up requirements.
- Prepare project summaries, presentations, reports, and educational materials.
- Work collaboratively with management and other team members to ensure projects are completed effectively and professionally.

Marketing & Community Engagement Support The Care Manager will also assist Modern Angels Healthcare with selected marketing and community engagement activities.

Responsibilities may include

- Attend community events, healthcare conferences, networking events, health fairs, and educational programs.
- Represent Modern Angels Healthcare professionally within the community.
- Assist with presentations to healthcare professionals, seniors' organizations, retirement residences, community groups, and referral partners.
- Help prepare PowerPoint presentations, educational materials, brochures, and marketing resources.
- Support community outreach initiatives.
- Assist with maintaining relationships with healthcare professionals and community partners.
- Help coordinate lunch-and-learns, educational presentations, health fairs, and community events.
- Assist with follow-up communication after networking and community events.
- Support social media and healthcare-related marketing content where appropriate.
- Assist with identifying potential partnership and community engagement opportunities.
- Work collaboratively with the marketing and management team on campaigns, events, and special initiatives.

Qualifications
- Current registration as a Registered Nurse (RN) in good standing with the College of Nurses of Ontario.




- Minimum 2 years of nursing experience in an acute-care setting.
- Minimum 2 years of experience in community healthcare, home care, retirement living, long-term care, or a similar healthcare environment.
- Strong nursing assessment skills and clinical judgement.
- Excellent understanding of nursing skills, care planning, client safety, and professional nursing practice.
- Ability to recognize changes in client condition and respond appropriately.
- Strong teaching and education abilities.
- Comfortable providing guidance and support to healthcare staff.
- Excellent verbal and written communication skills.
- Strong interpersonal and relationship-building skills.
- Comfortable speaking and presenting in front of groups.
- Professional and confident when communicating with clients, families, healthcare professionals, and community partners.
- Robust organizational and time-management skills.
- Excellent attention to detail and follow-through.
- Able to manage multiple responsibilities and competing priorities.
- Self-directed, motivated, resourceful, and able to work independently.
- Strong critical-thinking and problem-solving abilities.
- Comfortable taking initiative and contributing ideas.
- Strong computer skills, including Microsoft Office and PowerPoint.
- Interest or experience in community engagement, healthcare marketing, special projects, or business development is an asset.
- Valid driver's licence and reliable access to transportation for client visits, meetings, and community events.
