Join CMHA-YRSS as a dedicated Case Manager in Vaughan, ON, focusing on mental health and addictions recovery. Engage with clients to support their journey towards independence and wellness. This permanent, full time position on the Assertive Community Treatment Team requires you to work closely with clients 16 and over dealing with severe mental illness.

Your role will include setting recovery goals, facilitating access to services, and adhering to health and safety guidelines while traveling within the community. Key Responsibilities:
Comply with Intensive Case Management Services Standards
Manage a diverse caseload with significant travel requirements
Recognize trauma symptoms and provide corresponding assistance
Reference other CMHA or external services for client support
Support goal-setting through effective client communication Requirements:
Degree or diploma in health or social services
Active registration with OCSWSSW or similar body
At least one year’s experience in mental health settings
Proficiency in a second language is a plus
Valid driver’s license and proof of insurance Deliver compassionate mental health support and promote recovery for clients at CMHA-YRSS.
