This full time position focuses on patient-centered care, protected transitions, and comprehensive management in our Alternate Health Facility. As a driven and autonomous Nurse Practitioner, you will coordinate extensive care management, ensuring effective assessments and treatments.

Working closely with interprofessional teams, your role will promote innovative practices and patient advocacy, contributing to systemic improvements and best practice evidence. Key Responsibilities:
Deliver autonomous care across the full continuum of patient transitions
Collaborate with teams to enhance patient flow




Act as a primary point of contact for care coordination
Facilitate transfer of care when needed
Engage in leadership and education initiatives for nursing staff Requirements:
Active RN registration in Extended Class in Ontario
Master of Science in Nursing or equivalent qualification
Minimum of three years relevant clinical nursing experience
