Support oncology patients navigating their care journey
Facilitate connections with primary care providers
Create and evaluate survivorship care programs
Educate patients on available resources
Collaborate with multidisciplinary teams on patient needs Requirements:
Active Registration with the College of Nurses of Ontario
Five years of experience in oncology nursing
Completion of Chemotherapy & Biotherapy Course
Robust organizational and communication abilities
