As a reputable home health care provider in Ontario, CarePartners is currently looking for RNs and RPNs to work in its clinic located in Mississauga. This role is designed to promote patient well-being and enhance the care experience. The position requires a two-week rotation schedule, including weekends and on-call duties.
Key Responsibilities:
• Create and manage patient Care Plans with a community focus
• Conduct in-person nursing assessments and services
• Deliver a variety of clinical services in the clinic
• Collaborate closely with healthcare teams
• Undertake additional clinical tasks as assigned
Requirements:
• RN or RPN degree with active CNO registration
• Passion for patient care and community health
• Robust interpersonal and communication abilities
• CPR Certification is mandatory
• Updated immunization proofs and background documentation
