Provide crucial support as a Case Manager to 2SLGBTQIA+ youth in downtown Toronto. Direct community support focuses on mental health and crisis intervention within a hybrid work model. As a Case Manager, you will be responsible for assessing youth needs and developing tailored care plans.

This role emphasizes trauma-informed care, crisis intervention, and collaboration with various internal and external services. You will also facilitate workshops and advocate to ensure youth access essential resources in social services and housing. Key Responsibilities:
Conduct assessments and develop individualized care plans
