Role Description The Licensed Practical Nurse (LPN) Supervisor – Continuing Care / Home Care oversees and supports the clinical and caregiving team to ensure secure, high-quality care for clients in their homes. This full-time, on-site role in Edmonton, AB includes assessing client needs, developing and updating care plans, and coordinating services with therapists, physicians, and other health professionals. The LPN Supervisor provides clinical leadership by mentoring staff, monitoring performance, and ensuring adherence to professional standards, policies, and regulatory requirements.

Day-to-day responsibilities include conducting visits, supervising and guiding caregivers, managing documentation and reporting, participating in case conferences, and responding to urgent client needs.



The role also involves promoting client-centered care, educating clients and families on health management, and supporting continuous improvement in care delivery.

Qualifications

- Current registration and good standing as a Licensed Practical Nurse (LPN) with the appropriate provincial regulatory body in Alberta.
- Experience in home care, continuing care, or community nursing, with demonstrated ability to assess, plan, implement, and evaluate client care.
- Supervisory or leadership experience, including mentoring staff, providing feedback, and supporting performance and professional development.
- Strong clinical judgment and critical thinking skills, with the ability to manage complex cases and coordinate multidisciplinary care.
- Excellent communication and interpersonal skills for working with clients, families, caregivers, and healthcare partners from diverse backgrounds.
- Proficiency in documentation, electronic health records, and basic computer applications used in clinical and administrative tasks.
- Knowledge of infection prevention and control, medication administration, and relevant home care and continuing care standards and regulations.
- Ability to work on-site and in the community, including reliable transportation for client visits, and willingness to adapt to changing client needs.
- CPR/BLS certification and other relevant clinical certifications; previous experience in Edmonton or similar settings is an asset.
