Vancouver Island Health Authority is seeking a Primary Care Clinical Pharmacist to deliver comprehensive medication management within the primary care network in Victoria, BC. You will collaborate with an inter-skilled team to identify and resolve drug therapy problems, improving patient safety and outcomes.
As a key member of the team, you will work across multiple clinics, travel as needed between sites, and may use a personal vehicle to support scheduling and patient care delivery.
