The Health Centre in Waterloo Region is hiring a Nurse Practitioner – Outreach to support outreach teams serving individuals who are homeless or precariously housed. The role involves primary care within a mobile model and participation in hub locations. We seek an NP with Primary Care training, strong collaboration skills, and a commitment to social determinants of health. Hours include one evening per week and occasional weekends;
