Serving Muskoka and Parry Sound, the Alzheimer Society of Muskoka is dedicated to improving the quality of life for individuals living with Alzheimer’s disease, and other dementias, and their caregivers. We are committed to advocacy, public education and local services and programs. Position Purpose The Intake Coordinator will provide dedicated intake capacity to support referrals generated through NSM SGS personnel working in the Emergency Departments of Muskoka hospitals.

The primary purpose of the position is to ensure that individuals and families referred to the Alzheimer Society of Muskoka (ASM) through these hospital-based contacts are reached promptly, their needs are assessed, and they are connected with appropriate First Link services and community resources.

It is anticipated that approximately 25–30 referrals will be generated through the NSM SGS hospital-based initiative during the contract period.

When capacity permits, the Intake Coordinator will also support ASM’s broader intake process, helping to enhance responsiveness and reduce pressure on existing First Link staff.

Key Responsibilities

1. NSM SGS Hospital Referral Intake – Primary Responsibility

- Receive and monitor referrals originating from NSM SGS personnel in the Emergency Departments of Muskoka hospitals.
- Contact referred individuals and/or family caregivers in a timely and compassionate manner.
- Complete an initial intake and information-gathering process to understand the individual’s circumstances, needs and priorities.
- Provide information about ASM programs, First Link services and other appropriate community resources.
- Determine appropriate next steps and facilitate connections to ASM services.
- Make appropriate referrals to other health, social service and community agencies when required.
- Document all contacts, referrals, outcomes and follow-up activities in the appropriate ASM database/system.
- Follow up with individuals and families as appropriate to ensure that connections to services have been successfully established.
- Maintain communication with the appropriate NSM SGS contacts regarding the referral process,



while respecting privacy and confidentiality requirements.

2. General ASM Intake Support

When NSM SGS referral activity does not utilize the Coordinator’s full available hours:

- Assist with ASM’s general First Link intake process.
- Respond to incoming inquiries and referrals from individuals, families, healthcare providers and community organizations.
- Conduct preliminary intake and needs-assessment conversations.
- Assist with triage and connection to appropriate ASM staff and programs.
- Support follow-up on outstanding referrals and waitlists.
- Help maintain accurate and timely intake records.
- Identify recurring needs, service gaps or barriers encountered by individuals and families and bring these observations to the attention of the Program Manager.

3. Administrative and Program Support

- Maintain accurate, confidential and timely records.
- Participate in staff meetings and relevant training.
- Contribute to tracking referral volumes, outcomes and service connections.
- Assist with preparation of basic statistics and reports related to intake activity.
- Follow ASM policies, procedures and privacy/confidentiality requirements.
- Perform other related duties as assigned.

Qualifications

- Experience in a healthcare, social service, community service, intake, case management or client-service environment.
- Excellent interpersonal and communication skills.
- Ability to engage respectfully with individuals experiencing dementia and their caregivers.
- Strong listening, assessment and organizational skills.
- Ability to work independently while maintaining effective communication with a small team.
- Comfortable working with databases and electronic documentation systems.
- Understanding of dementia, caregiver needs and community-based services is an asset.
- Knowledge of Muskoka health and social service resources is an asset.
- Valid driver's licence and access to transportation would be an asset.
- Ability to reliably commute to our Bracebridge, ON location (P1L 1S2) for this in-person position

Performance Expectations

During the contract, the Intake Coordinator will be expected to:

- Establish an effective and reliable intake process for NSM SGS Emergency Department referrals.
- Ensure that approximately 25–30 anticipated NSM SGS referrals receive timely contact and appropriate follow-up.
- Ensure referrals are accurately documented and outcomes tracked.
- Improve ASM’s overall intake responsiveness by providing additional capacity when NSM SGS referrals do not require the Coordinator’s full hours.
- Provide useful information to ASM regarding referral patterns, client needs and potential service gaps.
- Maintain appropriate documentation to support reporting to NSM SGS and ASM.
