Ontario Health at Home in Ottawa is seeking a bilingual Hospital Care Coordinator to assess patient needs, determine eligibility, arrange service planning, and coordinate with families and health care partners. You will work across office, community, or hospital settings to support secure hospital-to-home transfers.

You will be responsible for developing care plans, coordinating resources, and ensuring timely discharge planning, with a 35-hour work week and a starting salary of CAD 44,480–47,517.
