Key Responsibilities:
• Conduct detailed in-home assessments and mobility evaluations
• Design personalized, goal-oriented treatment strategies
• Monitor client progress and adapt care plans as necessary




• Coordinate with healthcare providers for comprehensive support
• Educate clients about safety and independence strategies

Requirements:
• Registered in Ontario with the College of Occupational Therapists
• Experience in community/home care preferred
• Proven skills in assessments and treatment planning
• Ability to manage documentation effectively
• Valid driver’s license and reliable vehicle required
