This role emphasizes collaboration with healthcare teams and supporting self-management through personalized education in both group and individual settings. Key Responsibilities:

- Develop and assess individualized care plans alongside healthcare providers
- Facilitate education sessions for diabetes self-management
- Maintain accurate records of patient care and teaching evaluations
- Contribute to case reviews and educational resource development
- Support new staff through orientation and mentoring Requirements:
- Valid registration as a Registered Nurse with BCCNM
- Two years’ relevant clinical experience in diabetes care
- Certification in CPR and as a Diabetes Educator preferred
- Excellent interpersonal and communication skills
