The Eagle Urban Transition Centre seeks a full time Indian Day School Advocate to assist First Nations families in accessing vital services. This position focuses on ensuring that families, especially those with special needs, get the necessary support and resources. You will work directly with First Nations youth and families under the guidance of the Service Delivery Director.

The role involves working independently and collaboratively to provide referrals and assist in developing care plans. You'll communicate vital information on health interventions and accessibility, maintaining the utmost confidentiality. Key Responsibilities:

- Help families navigate provincial and federal resources
- Aid clients in resolving issues through proper referrals




- Communicate care plans and educational needs to families
- Collaborate and refer clients to relevant service providers
- Maintain accurate and confidential case management files Requirements:
- Post-secondary degree in Social Work or equivalent training
- Experience within the child welfare or First Nation context
- Knowledge of the Personal Health Information Act
- Strong organization and communication capabilities
