The HBHC Worker will support Indigenous families with children from birth to age six by helping them access health and social services. This role involves building trusting relationships, conducting home visits, and offering culturally appropriate care that addresses physical, emotional, mental, and spiritual well-being. The worker will collaborate with families to develop personalized care plans and connect them with community resources to promote healthy child development.

Responsibilities

- Conduct screenings and assessments

- Provide regular home visits and follow-ups

- Develop and implement family-centered care plans

- Advocate for families with service providers

- Facilitate referrals to health, social, and community services

- Build partnerships with local agencies and service networks

Qualifications

- Diploma or degree in Health or Human Services

- Experience in community-based programming

- Understanding of Indigenous cultures and traditions

- Solid communication and interpersonal skills

- Valid driver's license

- Clear Criminal Record Check (CPIC)
