Durham Community Health Centre (DCHC) is seeking a Case Manager, Welcoming Streets to provide outreach and individualized case management using the Housing First approach. The role targets unsheltered clients and connects them with housing supports, health and social services.
The successful candidate will work within an interdisciplinary team, travel within Durham Region, and adapt to client needs in a agile community setting. This is a one-year contract, with evenings/weekends as required.
