In this role, you'll work alongside a multi-disciplinary team to create individualized care plans and ensure the implementation of services for adults with intellectual disabilities.

You'll advocate for client needs while managing a smaller caseload, enabling you to provide meaningful support and monitor service effectiveness. Key Responsibilities:

- Identify client needs and develop tailored care plans
- Coordinate and advocate for identified services
- Track progress and assess service quality
- Collaborate with the multidisciplinary team for optimal outcomes
- Offer guidance for social work licensure support Requirements:
- BA/BS in human services or similar discipline
- Minimum 1 year experience in the IDD sector
