The Aboriginal Peoples Alliance Northern Ontario is seeking an HBHC Worker to support Indigenous families with children birth to age six by connecting them to health and social services. You will conduct home visits, build trusting relationships, and provide culturally appropriate care addressing physical, emotional, mental, and spiritual well-being.
You will collaborate with families to develop personalized care plans and link them with community resources to promote healthy child development,
