William Osler Health System is seeking a Palliative Care Clinical Coach to improve access to palliative care across Central West region, collaborating with CW OHT partners and Osler’s palliative team to drive practice change.
The Clinical Coach delivers education, coaching and mentoring to priority organizations, supports implementation of the Ontario Health framework, and contributes to quality improvement and data reporting.
