Providence Care is seeking a dedicated Social Worker to provide psychosocial assessments and support in a full-time capacity. In the role, the Social Worker will report directly to the designated manager and will be responsible for case management, discharge planning, and family education.

- Conduct assessments using evidence-based methods

- Master of Social Work (MSW) degree
- Current registration with Ontario College of Social Workers
- Experience in hospital Social Work (two years preferred)
- Proficiency in electronic patient record systems preferred
