The Registered Nurse (RN) will provide safe, evidence-based and client-centered nursing care in client homes and community settings. The RN will independently assess client, develop and evaluate care plans, administer medications and treatments, manage complex conditions, provide education and coordinate care with healthcare professionals and families.

Key Responsibilities

- Conduct comprehensive nursing assessments and monitor changes in health status.
- Develop, implement and evaluate individualized care plans.

- Administer medications, injections and prescribed treatments safely.

- Provide wound, catheter, continence, pain and chronic disease management.
- Manage complex, palliative, post-operative and rehabilitation care as assigned.
- Educate clients and families regarding medications, treatments and disease management.
- Recognize clinical deterioration and initiate appropriate interventions/escalation.
- Coordinate care with physicians, RPNs, PSWs, therapists and other healthcare providers.
- Maintain accurate clinical documentation and electronic health records.
- Follow CNO standards, infection prevention and control, privacy, safety and employer policies.

Required Qualifications & Licenses

- Current registration in good standing with the College of Nurses of Ontario (CNO).

- Diploma/BScN in Nursing or equivalent.





- Previous nursing experience in Home Care, Community Care, Rehabilitation or a related clinical setting.

- Solid clinical assessment, care planning and documentation skills.

- Demonstrated ability to work independently and manage multiple client needs.

- Experience with medication administration, chronic disease management and patient education.

- Excellent communication, interpersonal and collaboration skills.

- Current BLS/CPR certification.

- Valid driver's licence and reliable access to transportation, as required for community visits.
- Proficiency with electronic documentation/EMR systems.
- First Aid/CPR where specified by the employer.
