This position is a shared role between the primary care clinic and the regional diabetes education program at RCHS, Brockville site. Within the primary care clinic, the Registered Nurse (RN) provides direct client care, nursing services, health education, and care coordination to individuals with complex needs within a multidisciplinary primary care environment. The RN independently leads and delivers specialty clinics and provides clinical support to the primary care clinic as required to ensure continuity, accessibility, and safety of care. This role performs clinical and coordination functions to support the delivery of comprehensive, client-centred primary health care. Within the diabetes education program, the RN works alongside a team of RNs and registered dietitians to provide community-based nursing focused on diabetes education and management, and participates in the development, delivery and evaluation of programs and services for individuals and families that are affected by diabetes in Lanark, Leeds and Grenville.
