Registered Practical Nurse - Integrated Medicine and Rehab Services Job Number: J0926-0179 Job Title: Registered Practical Nurse - Integrated Medicine and Rehab Services As a member of the interprofessional team, the Registered Practical Nurse (RPN) on the Integrated Medicine and Rehabilitation Services (IMRS) unit provides patient-centered nursing care to support the assessment, planning, implementation, and evaluation of care for assigned patients. The RPN practices within their scope in accordance with the College of Nurses of Ontario standards, Hospital policies, and Patient Care Services guidelines. Care is delivered in a collaborative inpatient environment to support safe, high-quality care, continuity of care, and positive patient outcomes.

Key Responsibilities - Meets the College of Nurses of Ontario Standards of Practice for Registered Practical Nurses and practices in accordance with Hospital and Patient Care Services policies and procedures.

- Organizes,implementsandevaluatesnursing care plansto meet theassessedneeds of each assigned patient.
- Performs nursing interventions including vital signs, aseptic techniques and sterile dressing changes, infectionpreventionand control practices,monitoringnutritional intake, and specimencollection - Administersmedication,monitorstherapeuticeffectsand documents patient responses accurately and in a timely manner - Providespre-operative and post-operative personal and comfort careasrequired.
- Utilizes criticalthinkingto assess changes in patient condition andinitiatesconsultationor transfer of care as appropriate - Supports patient and family education and participatesin assessing health needs and promoting health maintenance.
- Provides cognitive, social, emotional, and spiritual supports to patients andfamilies - Utilizeshuman and material resourceseffectively and responsibly.
- Participates in quality improvement activities, audits, and program initiatives to enhance patient safety and care outcomes.
- Participatesin the orientationand mentorshipof newteammembersand nursingstudents, including preceptorship activities as assigned.
- Assistsinmaintainingthe established standards of care through individual performance, participation inqualityimprovementinitiatives, audits, andhospital committeesasrequired.
- Maintains professional competence through ongoing learning, participation in education sessions and in-services, and self-assessment of professional development needs.
- Responsible forthe safety and welfare of patients.
- Functionsin accordance withestablished hospital emergency procedures.
- Promotes effective communication and collaborationwithin and across departments.
- Participatesas a member of theinterdisciplinary team,promoting effective liaisons between patients, family and physicians, healthcareteammembers,and community agencies. Encouraging referrals toappropriate communityresources.
- Maintains awareness of current developments in nursing practice and relevant legislation, including the Regulated Health Professions Act (RHPA).
- May be redeployed to other patient care areas based on patient care needs and operational requirements.




- Performs otherrelatedduties as may be assigned by the Program Manageror designate Qualifications Education - Registered and in good standing with the College of Nurses (CNO) as a Registered Practical Nurse is required.
- Current Basic Cardiac Life Support (BCLS) isrequired; CPR Level HCP (Health Care Provider) willbe accepted as an equivalent.
- Completion of Gentle Persuasive Approach (GPA) training or willing to complete is required.
- Certificate in Gerontology and/or Palliative Care from a recognized college, is considered an asset.
- IV certification or the ability to obtain certification within six (6) months of hire.

Experience with IV initiation may be considered.

Experience - Minimum two (2) years acute care experience is preferred.

- Minimum one (1) year of experience in Rehabilitation, Seniors Care, and/or Palliative Care, and/or previous work experience on an Acute Stroke Unit or Integrated Stroke Unit is preferred.

Skills and Traits - Must be able to meet the physical and emotional responsibilities of the role, including standing and walking for extended periods, frequent and repetitive pushing, pulling, bending, and lifting up to 50 pounds, as well as safely operating equipment.

- Ability to meet the physical demands of the position, as outlined in the Physical Demands Analysis (PDA).
