Fraser Health is seeking an Occupational Therapist to work in a home-based unit, linking patients with care staff via technology from a central office. You will assess, plan, deliver, and monitor OT services, including community outreach and virtual care to patients in their homes.

You'll collaborate with mental health teams, educate patients and families, and contribute to program development while maintaining records and participating in ongoing qualified learning.
