About the Role

Assistsindividuals to live successfully in residential settings such as group homes. Ensures that individuals’ physical, emotional, social, educational, and medicalneeds are met.Assistsindividuals to enhance quality of life with activities of daily living and the development of life skills.

Responsibilities

- Participate in the development, implementation, and evaluation of individualized support plans.
- Provide person-centred support to promote independence, life skills development, community inclusion, and overall well-being.
- Assistwith activities of daily living, personal care, medication administration, and specialized care asrequired.
- Support social, recreational, educational, and vocational activities
- Respond to emergencies, behavioural challenges, and health and safety concerns.
- Monitor,



document, and report on individual progress and program outcomes.
- Coordinate with families, professionals, and community resources to support individual goals.
- Provide transportation, household support, and other related duties as assigned.
- Certificate in a related Human Services, Social Services, Community Support, Disability Support, or equivalent fieldis an asset.
- Minimum one (1) year of recent related experience.
- Equivalent combinations of education, training, and experience may be considered.
- Strong communication, interpersonal, and problem-solving skills.
- AClass 5 Driver’sLicense isrequiredas a condition of employment.
- Current First Aid/CPRand foodsafecertificationisan asset.
- Licensed homes require a negative TB test result
- Must be legally able to work in Canada
- High School Diploma or equivalent
