HammondCare is seeking 3 x 0.6FTE Registered Nurses to join our Home Care Clinical Care & Practice Team. This permanent part time Clinical Partner – Nursing role covers the Central West/Nepean/Blue Mountains region, with 3 days per week between Monday and Friday.

You will work closely with an experienced multidisciplinary team to provide tailored services to older adults living at home. A typical day includes assessing care needs, setting goals, delivering treatment and interventions, and
