Become a Health and Housing Support Coordinator at Guelph Community Health Centre. This full time role focuses on assisting vulnerable individuals in overcoming barriers to health and housing stability.
In this impactful position, you will be part of the Allied Health Team, engaging with residents of the Permanent Supportive Housing Program. You will advocate for clients, navigate healthcare services, and coordinate between various support systems to promote wellbeing and stability.
Key Responsibilities:
• Guide clients in navigating social services and healthcare
• Help clients with care plan engagement
• Advocate for necessary services within the healthcare system
• Coordinate internal and external referrals for client support




• Facilitate case management with interdisciplinary teams
Requirements:
• Post-secondary education in social work or similar field
• 3-5 years’ experience in community organization settings
• Excellent advocacy and interpersonal capabilities
• Understanding of health and social service integration
• Valid Class 'G' Ontario Driver's License required
