Your time with us can accommodate career growth as you advance your education and qualified credentials.Key ResponsibilitiesAdvocate for First Nations clients affected by diabetes and related health conditions.Plan, deliver, and evaluate diabetes care across the lifespan, providing education, screening, testing, and follow‑up in clinic, community, and home settings.Review medical histories, maintain accurate EMR documentation, and use data to inform care planning.Provide individual and group diabetes education using culturally appropriate and diverse learning approaches.Collaborate and communicate with primary care providers, mental health teams, and community partners to coordinate care and share significant findings.Act as a resource to staff and community members, promoting diabetes awareness through training, events, and outreach.Compile program data, support quality improvement, and follow Diabetes Canada Clinical Practice Guidelines and professional practice standards.Participate in ongoing learning and professional development; perform additional duties as assigned.Job QualificationsKnowledge of First Nations culture (Carrier and Sekani preferred) and diabetes management.Solid communication, organization, teaching, and documentation skills.Able to work independently and within a multidisciplinary team.Proficient with Microsoft Office, EMRs, and common office equipment.Registered RN, LPN, or RD with current registration; Diabetes Educatorcertification or willingness to obtain.Experience in clinical care or cross‑cultural health promotion.About UsFor over 40 years, proudly helping healthcare workers take control of their career.Carecor Health Services Ltd.
