Become a Mental Health and Addictions Outreach Nurse with Vancouver Coastal Health, focusing on client care in Vancouver, BC. This role offers the prospect to support individuals through crisis intervention and outreach services. As a Mental Health and Addictions Outreach Nurse (RN/RPN) with Vancouver Coastal Health, you will work as part of the Assertive Community Treatment Team. Your focus will be on providing inclusive services for clients with mental health and substance use challenges.

Key responsibilities include mental health assessments, crisis intervention, and developing integrated care plans in collaboration with an interdisciplinary team. Key Responsibilities:
Conduct mental health and physical assessments for clients




Provide crisis intervention for high-risk situations
Collaborate in developing and adjusting care plans
Facilitate client movement between care settings
Attend case conferences with interdisciplinary teams Requirements:
Current RN/RPN registration with BCCNM
Two years' experience in mental health and addiction
Valid BC Driver's License and personal vehicle access
Robust knowledge of crisis intervention practices
