As a Float Hospital Care Coordinator, you will play a key role in developing comprehensive care plans tailored to individual patients, facilitating a smooth transition from hospital to home.

Engage and assess clients for healthcare program admissions




Establish care plans based on client assessments
Implement and monitor client care initiatives

Manage discharge planning and resource redirecting Requirements:
Degree in Nursing, Physiotherapy, or Social Work required
At least two years of related healthcare experience
Knowledge of local health care delivery systems
Expected COVID-19 vaccination as per policy
