Clinical Coordinator – EXTENDICARE GUILDWOOD
Full‑time day (hours may vary depending on the operational needs of the home).
Works closely with the DOC to ensure the Nursing Department operates in accordance with MOHLTC, CNO, and Extendicare standards and policies.
Acts as the lead in the coordination and implementation including evaluation
Acts as the Co-Lead Nursing Educator, leading and delivering staff individual and group training as it relates to special functions and routine practices.
Acts as resource to the interdisciplinary team in their interactions with residents and families
Supports resolution processes as identified through policies and procedures
Provides constructive feedback to the Director of Care regarding staff performance and suggests staff training and education needs
Complete RAI-MDS (RAP, Coding, Care Plan etc.)



Bachelor of Science in Nursing or equivalent post diploma education preferred
A minimum of 3 years’ experience in clinical nursing in seniors’ care that may include geriatric, long-term care, rehabilitation or chronic complex care
Current registration with the College of Nurses of Ontario and a member in good standing with applicable regulatory body
attendant service delivery work setting supporting the activities of daily living for residents
