PSW-ICCP significantly assists in enabling individuals to live in a safe and healthy manner after being discharged from hospital. The PSW-ICCP works cooperatively with the ICCP Sr. Manager, Supervisor, Program Assistants and other care givers in providing quality client-centered care, and in the effective execution of daily tasks. PSW-ICCP adheres to daily care schedules and overall care plans. ICCP clients have recently been discharged from many different pathways and diagnoses. These clients required more support in the daily management of their own care needs and daily living activities for a short period of time.
