You will be tasked with providing health education, improving wellness, and managing chronic conditions for elderly clients. Key Responsibilities:
• Provide holistic health care services to clients
• Manage clinics focused on chronic disease and health promotion
• Lead health education programs within the community




• Develop client-centered care plans in collaboration with the team
• Participate in quality improvement and team development activities

Requirements:
• Registered with the College of Nurses of Ontario
• Bachelor of Science in Nursing required
• One year of related experience in community care
• BCLS certification and vehicle access for travel
• Strong communication and problem-solving abilities
